Return Product Authorization (RPA) Form

To submit a Return Purchase Authorization (RPA) request, complete the following form. In order to expedite your request, please complete all information requested below.

Return the completed form to sales@mcbridehealthcare.com or fax to 410-216-0345. You will be notified if your request has been approved.

Please provide specific shipping instructions for this order. If you do not provide shipping instructions, we will ship the order BEST WAY, prepaid, and add the shipping charges to your invoice.

Product Information

Product Returns for “Warranty” Determination

McBride Healthcare, LLC will conduct product testing in accordance with the problem description listed on Page 1 of the RPA request form. If, after evaluation, the problem description matches that on Page 1 of the RPA request form, the product will be replaced at no cost and dispatched back to the customer prepaid. We will contact the consumer if the problem description differs from the problem listed on Page 1 of the RPA request form or if the product was damaged during delivery. We reserve the right to charge for return shipping if the product has no discernible defects.

No Warranty Credits or Exchanges for:

• Returned items that failed as a result of an accident, purchaser maltreatment, neglect, or failure to operate according to the owner's manual(s) supplied.
• Returned items that failed because of improper voltage or wiring.
• Items that failed due to precipitation, excessive humidity, corrosive environments, or other contaminants.
• Any product damaged during shipping.
• Any product failure resulting from installation or operation not in accordance with the installation and operation instructions, or damage caused by contact with tools or the environment.
• Items returned with cosmetic flaws that do not affect product functionality.
• Items that are returned incomplete or defaced.
• All consumable or standard wear items, including fuses and incandescent lighting.
• Items returned with a different serial number than what was authorized.
• Items returned that were specially ordered or configured.
• Freight damaged products. If your shipment arrives damaged, you must document the damage on the carrier's delivery record in accordance with the carrier's policy, save the merchandise in its original box and packaging, and arrange for a carrier inspection of damaged goods.

A 20% reclamation fee and round-trip shipping costs will be deducted from the credit refund for any product returned to McBride Healthcare, LLC for reasons other than warranty. All returned items must be in their original packaging and include all manuals, accessories, and packing materials.

Please ensure that your return is packaged meticulously. McBride Healthcare is not responsible for shipping-related product damage or loss. Any damage or subsequent failure of the hardware product caused by improper packaging will incur additional fees for product repair.

If the consumer requests an expedited return, the item will be shipped collect.

Product Returns for “Non-Warranty” Determination


After McBride Healthcare's evaluation, the consumer will be informed of the cost of the repair. We will contact the consumer if the problem description differs from the problem listed on Page 1 of the RPA request form or if the product was damaged during delivery. If we cannot identify a problem with the product, we reserve the right to charge for testing and return shipping.

The customer must then provide written authorization to proceed with the repair(s), agree to cover the costs of the repair and return shipping, or authorize the product to be returned as-is, at the customer's expense. If written confirmation is not obtained within thirty (30) days of notification, the product will be returned as-is at the customer's expense. The warranty period for repairs is ninety (90) days from the date of shipment.

Please ensure that your return is packaged meticulously. McBride Healthcare, LLC is not responsible for shipping-related product damage or loss. Any damage or subsequent failure of the hardware product as a result of improper packaging will incur additional fees for its restoration.
